2025-2026 Youth & Children Registration

We’re so excited to welcome your family back to programs and events at UPPC! To help us provide the best care and a safe environment, we ask that every parent/guardian complete a Youth & Children’s Registration for each child/student.

This registration is required before your child attends programming or events this year—including Sunday morning and Wednesday night activities. It collects important contact, medical, and emergency information and gives the necessary permissions for your child to be fully involved.
